Commonly used CSMAR Forms are below

Credit Card Authorization Form Used when authorizing credit card payment processing.  (Note: we do not keep credit card information on file.  This form is to be used on a per transaction basis.)

Member Information Change Form – Used when member’s information changes (e.g. Name, Home Address, etc)

Member Office Transfer Form Used when a member transfers from one office to another. New Broker’s signature required. (Note: there is a $40 transfer fee)

Office Severance Form Used when an agent leaves your office.  Broker signature required.

Office Information Change Form – Used when an office’s information changes. Broker signature required.  (e.g. Office Address, Office Phone, etc.)

CSMAR Change Authorization Form – Used when a change is needed on a listing (e.g. Status, Cancellation, Picture and Deletion, etc.)  Broker signature may be required on some changes.
