Member Information Change Form – Used when member’s information changes (e.g. Name, Home Address, etc)

Member Office Transfer Form – Used when a member transfers from one office to another. New Broker’s signature required. (Note: there is a $50 transfer fee)

Office Severance Form – Used when an agent leaves your office.  Broker signature required.

Office Information Change Form – Used when an office’s information changes. Broker signature required.  (e.g. Office Address, Office Phone, etc.)

Local Dues Waiver Form – Used when a member who has at least 30 years consecutive membership* with CSMAR is requesting his/her local dues be waived. (*other qualifying criteria applies)

CSMAR Change Authorization Form – Used when a change is needed on a listing (e.g. Status, Cancellation, Picture and Deletion, etc.)  Broker signature may be required on some changes.

Coming Soon Addendum – Used optionally to detail the Coming Soon status to your seller/housing provider.

IDX Approval Form – Used to request listing display on your site when working with an IDX vendor.

Clerical User Application – Used to authorize a non-licensee or non-active licensee access to your MLS for administration of your database. Broker signature required 

Team ID Application – Used to request an MLS Team ID and name. Broker signature required 

Limited Function Referral Office (LFRO) Certification Form – Used to certify REALTORS as referral agents who exclusively solicit/refer clients and customers. Broker signature required

Mortgage Loan Originator (MLO) Certification Form – Used to certify affiliated MLO licensees. Broker signature required
